i used the formula in the LM5116 datasheet (page 23) and used these values:
VinMAX = 80V
Vout = 12V
Ipp = 2A
fsw = 250000 Hz

I think the main cause is the layout. You can refer to our EVM layout, it's quite same with your requirement. LM5116-12EVAL Evaluation board | TI.com . Now you can try to add 5Ohm gate resistor to Q1 to see if any improvement.
