For TXB0106 ESD testing, we use other ESD standard. If you want to use translation in aotumotive, could you check TXS0104E-Q1 and TXB0104-Q1

Is the output on the A & B signals 0 to 12V? If so than you will need to use a voltage divider to make the singals from 0 to 5V. This is due to the level shift chip use on the inputs of TMDSHVMTRPFCKIT. The part number is TXB0106.

You are right. The signals are fine on the B side of TXB0106 , but are erratic on the A side. I think that is causing all the trouble. I have ordered a new chip for replacement and hopefully that will fix the issue.
