LMC6482 as a DIfferential amplifier for analog current sensing application, I have received a failure such that opamp 6482 IC, its series resistor 10K got failed along with shunt resistor also open.
Shunt open is due to excessive current, it is clear but I am interetsed to know the reason why series 10K resistor, OP AMP IC failed like its output is always + Vcc range.
Kindly suggest us probable reasons, how can I simulate same case at my in house lab

Hi Raj,
A voltage at the +input of OPAmp falling below the negative supply voltage or exceeding the positive supply voltage of OPAmp can barbecue the built-in ESD protection diodes clamping the input voltage to the supply rails of OPAmp.
A too high input voltage could be the result of a heavy inductive kick back, when the current through a high inductance is suddenly interrupted, example given because your 51R shunt resistor burned out due to a too high current.
As even the 10k resistance did burn out this must have been a very high inductive kick back of an enormous energy. Or is the catastrophic failure simply the result of the erroneous connection of shunt to a wrong voltage, like mains voltage or similar?
