OPA2675: Amplifier output noise

Hi,

I am using this as non-inverting amplifier with supply of +/-6V, 3.3V square wave input of 1Hz, with feedback resistors of 200Ohm (Both rf, r1 are 200ohm). 

In the positive cycle of square wave output is around 5V which is as expected, but when input is 0, we are getting tooo much noise at output as shown in below figure.

Please help us with this problem.
