This opamp has two V+ pins (and one V- pin). In order to bias it properly, do we need to connect the positive supply voltage to both V+ pins, or one can be floated/grounded?

Hello Nima,
The worst possible choice is to ground one of the V+ (unless the other V+ is also ground because input supply is negative only). This would short out the power source.
Based on the VSSOP bond diagram that only uses A1 pin as V+ floating A2 should be fine.
The best choice is to connect both A1 and A2 to V+ supply.
