INA210 damage issue

Hi,  I ran into an INA210 damage issue.  This happened to many of the boards.

In my application,  the INA210 is to measure the current through a shunt resistor 0.003ohm. The voltage presented on the INA+ and INA- is a PWM voltage(amplitude 11V, f=5Khz, Duty cycle=20%). There are two TVS diodes(breakdown voltage 24V) to protect the input of INA210. Many of the INA210 were damaged after the circuitry start to work. The failure symptom is the output of amplifier isn't reference value again if no current is applied. Why did this happen? The maxim voltage for INA210 pin is 26v in datasheet.
