LMH6505: Potential Damage?

Hi, I'm using this component as a VGA (gain driven by a DAC) and I'm not seeing anything on the output. I'm wondering if I've possibly damaged it.

Configuration: typical application with Rin=10K, Rg=374, Rf=3.74K. Vg limited in software to not exceed +2.3V. V+/- at +/-5V and Vin potential range of 0-5V.

Potential issues: I've found that at startup my +5V power turns on about 100ms before the -5V. During this time, the -5V rail sits around +0.8V and Vin sits at about 4.4V.  I have also driven Vin at around its max of +5V when +/-Vin is fully initialized. I don't see anything in the absolute maximum ratings saying explicitly that this will damage the device, unless it's exceeding the input or output current? Do you think this could be damaged or am I having a different issue? Thanks!
