OPA2333: OPA2333SHKJ

We have been using up OPA2333SHKJ from LOT# 2245B, 2244B and 2245A and found one of them with OUTA(U3-1) with 4.04V where it should be 0V.  We tested this one piece and +INA(3pin)/V-(4pin) was OPEN.

Can you check two things,

1.       first if there were any reported claims about OPA2333SHKJ with similar issues.

2.       Whether this issue could have been arisen after the shipment was made.

    All units undergo final test and thus it is not possible for OPA2333 to be open between +INA (pin 3) and (V-) and still pass the test.  Therefore, the question is how do you make the measurement?  Do you use an ohmmeter between pins 3 and 4 with all other pins floating? If so, there is ESD diode between the two pins which depending on the polarity of the meter connections may show as a finite impedance (forward bias diode) or very high impedance (reverse-bias diode or open circuit).  If the meter shows open circuit in both directions, this would mean that the ESD diode (and likely input stage) has been damaged either by over-voltage or ESD event.
