ISO224: Power supply for ISO224

Tool/software:

Here is a simplified drawing of the circuit being designed.

1. Input voltage to the Isolation Amplifier: -4V to +4V
2. Output voltage -4V to +4V. Gain 1
3. Isolation voltage 5000V RMS
4. Input frequency less than 100Hz 

Questions:

1. Does the ISO224 meet our requirements? If not, could you recommend another amplifier that does?
2. Can we get negative voltages at the output of the ISO244 by supplying only positive power voltages to it?
