According to simulation ,I'm getting required differential output voltage 1.2V. But my doubt is ,if we consider separate output voltage  OUTP and OUTN (as per graph shown below) it is sine wave starting from 37V to peak 37.6V.Why such large voltage seen at output.

2) Should we add & subtract common mode voltage 1.44V to peak voltage to get correct output value

    I think your probe measurements are not relative to GND2.  If you measure VOUTP and VOUTN relative to GND2, the average between them should equal the common mode voltage (1.44V) and the difference would be equal to your input times the gain (0.4V/V)
