OPA2544: Case connected to V– Supply?

Sorry for this confusion.

This device underwent a PCN in 2017, and the primary reason for this PCN was in relation to the die attach material becoming obsolete. 

I suspect the original die attach material was conductive, and the new die attach is far less conductive (I measure 25Mohm DCR on bench).

This device passed requalification for the new assembly materials, so there will be no functional difference from this materials change.
