TLC3704: TLC3704 USE？

How are the inputs of the other three comparators connected?

Please add the voltage at pin 5 to the oscilloscope trace.

Why is the the blue input below ground in the 15V supply waveform? VCM range only goes to -0.2V, blue VIL looks lower than that. 

The reference line of oscilloscope debugging can be adjusted by selecting the knob, please help to see if the output frequency is not correct, thank you

Change CH3 vertical range to 200mV/division. The high input is fine, the low level input voltage is very likely the cause of the undesired output waveform 

My input can only be 5V PWM square wave, to achieve 15V PWM square wave output through TLC3704, use 15V power supply, is this OK? If not, in what way can it be achieved?

Please check the amplitude and frequency of the waveform, the reference point can be adjusted, please see below according to the provided schematic diagram, 15V power supply, 5V PWM input, output 15V PWM waveform frequency is wrong?

Let's jump to the solution because the debugging advice was completely and repeatedly ignored. 

The frequency was wrong because the input goes below ground. TLC3704 is not phase reversal resistant. Most new comparators are phase reversal immune.
