Can you confirm your input test signal? Can you also confirm the quiescent (operating) current of the THS2630 if you are measuring on bench with a PCB?

What is the signal frequency of your input waveform?
Does the output behavior improve if you add more resistance to the output, such as replacing C72 with a 1kOhm resistor?
Are the feedback capacitors sized correctly? Why was 100nF selected?
