At below circuit, for gain simulation at 1231MHz(1118.75MHz ~ 1343.75MHz),
How does customer have to connect?

Hi there David,
The port assignments can be seen in the comments in the top of the snp file by opening it with notepad.

In this case, port 1 and port 3 are INP and INM respectively, and port 2 and 4 are OUTM and OUTP respectively.
However, if the customer will be using in single ended to differential operation, there is a slightly better S-parameter file they can use, which includes the INM termination directly in the test fixture. This is available in the s3p file contained at the following URL www.ti.com/.../TRF0208-SP under "TRF0208 S-parameters model" (Yes i know its not labeled very well to differentiate S2D vs D2D).
