OPA170: OPA170

Dear Sir/Madam,

I am using the OPA170 Op Amp and I have damaged around 5 of them so far. They work at the beginning then they just wont work and the power supply gets dropped down and I find the power supply terminals(V+ AND V-)  were shorted with the ground for some reason. I am wondering why is this the case actually? I am giving it current of 8mA and 15 Volts power supply so what is the problem with it??

    I read that you have +15V and -15V supplies. The 8mA reference is a bit unclear. Is the normal application current draw 8mA?
    My first round of questions are:
    1) Are any of the input pins (or output) pin connected directly to board ground?
    2) Does the issue occur while running or during power up (or down)?
    3) Do the power supplies overshoot 15V or become opposite polarity voltage during power up or power down?
    4) Are safe ESD procedures being followed?
    5) Does the OPA170 get hot while running?
