Question 1 on p.16.
You are correct. It should be R2 in fo = 1/(12*pi*R2C). Reasons are:
At DC and low frequency, Vout/Vin = -R2/R1.
When R2 and Xc's impedance are equal, the gain is starting to decrease at -3dB point, which it generates the pole. R2 = 1/sC or fp = 1/(2*pi*R2*C).
Question 2 on p.19.
You are also correct. R1 and R4 should have been switched, see the image below. You can solve the bottom circuit by applying superposition method.
