LMH6703: Differential to single ended conversion, <=500MHz

Hi 

I need to convert the following signal from differential to single ended:

1) BW: 0.1MHz-500MHz (AC coupled)

2)Output signal level: ~100mVrms

3) Output HD2, HD3, IM3 <50dBc @BW

4)Gain 2-3 V/V

Can you suggest any of your devices for this task?
