Hi TTGtt,

you violate the common mode input voltage range of OPA188. Your input voltage must stay 1.5V below the positive supply voltage.

Hi TTG,

As Kai indicated, Vcm is outside of input voltage range, see the capture below. You have to increase Vcc or lower the input signal amplitude (remove C2 or place after R1, if this is intended).
