THS3491: IC blows up when powered.

Hello.
I'm having trouble with the THS3491 IDHAR, since it blows up when powered. I already bricked two of them. I bought them from Digikey (296-51646-1-ND).
The board works, so it's not a problem of power polarity: I soldered a TL081 (see photo below) and it outputs the corresponding pulse (a really ugly one since the speed difference of the ICs is huge). The two THS got the power pins shorted to ground.
The intended circuit is shown in the second image: it's a differential amplifier with gain of 10. The input pulse is 1 Vmax and 5 ns to 200 ns wide.
Maybe it's the fact that the THS3491 is a current feedback and I'm missing something.
