THS4051: Push Pull amplifier to generate high voltage

Going off the electrosurgery training here I am a little lost on how the RF Amplifier portion works (I've looked up regular push pull amplifiers that are only transistor based that don't include op-amps) and I've even looked at AN-272 as well. In addition, I can't seem to find any equations that would show how the final gain is calculated for this particular amplifier. (I started with a google search of op amp with transistor voltage booster which lead me to that AN-272 document)

You realize that app note is from 1981 - there are off the shelf parts to do this now,

TI has the OPA5xx series while Apex Microtechnology has a wide range of high V opamps.

Just checked out Apex Microtechnology found the MP118 to start with. It has a higher supply then the TI ones. Wouldn't have looked to check Apex otherwise.
