TINA/Spice/LMH6553: Weird simulation result with the configuration given by datasheet

I'm using this amplifier to do a driver an ADC but got some strange result on ADS. (using SPICE model) The VO+ and VO- is not on the Vocm is set (0.9v). 

So, to check my design, I isolated it and run a simulation in TINA, it's still not working. 

Finally, I "copied" exactly the design from datasheet and here is what I got. (the same design as Figure 1. Single-Ended Input Differential Output ADC Driver in LM6553 datasheet).

1. the gain of this configuration should be 1, and it's obvious not.

2. there is some offset on the output, as you can see, 45.64m.

3. the differential output looks not "so differential“.
