Are the input offset parameters that are specified in the datasheet +/-150uV and +/-5uV/V?

    Regarding to the input offset voltage, Yes, the max. Vos is +/- 150uV (typical is +/-40uV). The max. input offset voltage in dVos/dT vs. temperature is +5uV/C.

    I think that dVos/dT stated as positive is: For a given part, the Vos either increasing with temperature (e.g. positive temperature coefficient) or Vos decreasing with temperature (e.g. negative temperature coefficient). In either cases, dVos/dT will remain positive.
