PCM1860-Q1: explanation of the input mux

Table 1 in the datasheet describes the HW pin settings that produce different channel routing.

First, we’re receiving differential signals.  The table seems to imply that reading Ch 3 and Ch 4 always introduces +12dB Gain re: Ch 1 & CH 2.  Is this correct?  Or is there a way to sample Ch 3/4 at 0dB Gain?

Second, how fast can the MUX operate?  If we sampled at 96kHz, could the mux switch between inputs on every sample?
