DRV135: DRV135

I have another DRV135 audio question.  We're apply 6 Vpp at 1kHz with an open load on the two output pins (The circuit that we're using can be seen below).  The driver keeps getting damaged after using it  few times, could this be caused by the open load? Too high of a stimulus?

Does anyone have any ideas on what could possible cause damage this driver after running it a few times?

    The only possibility that I can see that would cause damage is if there are transient spikes that are creating and over voltage/current situation back into the device. Are you observing any transient spikes on any of the pins, supply lines, inputs, outputs? You will need to check this with an oscilloscope to confirm. There is no issue running the device at +/-15V with a 6Vpp signal. My recommendation would be to eliminate any long cables to debug and make sure that the device is operating normally under a basic setup in your application. 

    If you are observing transient spikes with cables or anywhere in the circuit you may need to add transient voltage suppression onto the pins. This can be done with TVS diodes. We have a training on such events that can be found here:

    I do not see any reason the device would get damaged in a basic setup as you have shown. The only way for us to help debug is with measurements to show the behavior on the pins to see if we observe any unusual behavior. If you can provide oscilloscope captures of the pins it would be helpful for us to assist in debugging.
