TAS5751M over-voltage, under-voltage, over-temperature caused by the chip protection

Hi, I am using TAS5751MDCAR, the board is basically designed and layout according to EVM board, the audio signal output is normal. But there are some  problems in the process of using. As follows

When adjusting the frequency and gain of EQ, the analysis with AP instrument is correct, but after I use the software to quickly drag the EQ control, it is easy to appear the chip is protected state. Once the chip enters the protected state, try to re-power the chip, the chip's protected state still can not be lifted. The value of Error Status Register is always 0x02 when read.
We monitored the PVDD voltage is normal, the chip temperature is very low, so the guess is not over-voltage, under-voltage, over-temperature caused by the chip protection. After we replaced the chip, the audio output of the board is normal.

We need to request answers to the following questions:
1, after quickly adjusting the EQ, the chip into the protection of the reason.
2, the chip into the protection state, restart the device can not be lifted inside the chip protection state.

Here is our pcb schematic, please help analyze whether it is reasonable.
