PCM1754: Speaker is damage (used speaker is OSR15R-4.8F1.2W8A)

Our product is biometrics device. We are using this circuit with OSR15R-4.8F1.2W8A speaker. We have a problem if device is continuously beep sound at regular interval for up to 48 to 72 hour , speaker is damage. Please find out circuit as below. What are the  possible chances for damage speaker?

    If you are sending the continues beeps at your will  to the system and you see the analog output as expected from the DAC, then the DAC is doing its job. 

    However, If under your above specific conditions, the speaker is getting damaged , you might want to investigate the power that is delivered to the speaker via the amplifier. For this specific  DAC we are using OPA1678IDR on our EVM . The Datasheet suggest  OPA2353 or OPA337 as well.

     I do not think the issue is the DAC itself because the DAC just convert the digital input to analog format and does not have any gain in its operation and the max voltage is limited per its spec. Le me know if you need any help with the DAC as I am the support engineer for our DACs.
