LME49724: Works for some time, then goes crazy

Hi,

I am currently working on a DAC reconstruction filter using the LME49724 after coming across massive issues using the OPA1632. 

The LME49724 worked fine on the first prototype. 

I ordered some more LME49724s from mouser for the second prototype, using the exact same circuit and the same layout. The only change is that a thermal pad was added to the footprint.

Unfortunately, the LME49724 behaves normally for a short period of time (seconds) and then goes completely crazy; meaning it draws about 4 watts per rail and does not produce any output.

There are no signs of oscillation visible on the output. 

Of course I have tested for shorts and tripple checked the component values, but I cannot find an explanation for this phenomenon.

Any help would be appreciated.
