TAS5630B: Have output Short to GND issue

Dears,

We use TAS5630BPHDR to drive 2 speaker, during the using,some pcs occur damage.

we test the phenomenon is:

1. TAS5630B's four output PINS(OUT_A/B/C/D),one of them short to itself GND(GND_A/B/C/D).

2. PIN-17(/SD) assert to high.

3. PIN-19(READY) have no output.

PVDD is 42V,

every speaker is 8ohm/150W;

PWM frequency is 400KHZ;

the output mode is BD MODE.

We made almost 500 pcs using TAS5630B and 50 pcs has this problem.

I could not understand.

How can I debug??

Is the device damaged when outputing high power? Could you provide the layout?
