Hi Kevin,
Nothing stands out to me in the schematic. Are you only seeing this on the inputs labeled as line inputs (Pins 14 and 16)? Or are you seeing this on all of the input pins? If you are only seeing this on pins 14 and 16, have you tried removing C840 and C841 and probing the input pin directly to see if the bias voltage is still below it's expected value?

Hi Kevin,
I see that you are using +48V power. Is your system requiring phantom power?
Can you confirm if the abnormal bias voltage is coming from the inputs connected with +48V power? If so, transients from the +48V may be making its way to the inputs of the CODEC and freezing up the internal circuitry.
Regarding the schematic, are R32 and R34 still not connected?
