LM4871: schematic review and heat issue

1. below is the schematic of LM4871. please help us to review it.

2. Issue.

1) Issue 1 :The IC temperature is geting hot and there generator a 2V(600kHz) noise.

2) Issue 2: no 2V(600kHz) noise, but IC is hot.

when change to a new device. issue is solved. Do you think it is a device issue ?
