TAS5631B: Going into shutdown mode for no apparent reason

I'm having an issue where the chip (TAS5631B) goes into shutdown mode. According to the datasheet, the only conditions for going into shutdown mode would be "under-voltage" or "over-current". I know it's not under-voltage because I'm measuring the voltage rails and it is stable. Above 47V for the PVDD rails, and above 12V for the GVDD rail. I also think it is not over-current, because I even disconnected the output filters, having the output open circuit, and even then it went into shutdown mode.

I've tried different startup sequences as well, doesn't seem to have any effect. The strange thing is that I got it working once or twice (with a load, even playing music through it), but I'm not able to recreate those conditions.

I'm using a TAS5508C as a source of PWM, and the READY signal on the TAS5631B stays high, so I assume my PWM signal is valid. I am using AD modulation, as this is the only modulation that the TAS5508C allows.

Providing no input, thus just a 50% duty cycle PWM signal, no difference is seen. As soon as I apply HIGH on nRESET, nSD goes LOW.

What other conditions would force the TAS5631B into shutdown mode? Any tips or ideas of how I can solve this problem?

    Have you tried a second amp or board? Just trying to crosscheck here.

    Our amps should not be operated without their output LC filter, no load is fine.

    You might be seeing PPSC if the fault condition shows right on startup. This means that there may be a short from one of the outputs (OUT_X) to PVDD or GND. I would check this with a meter to be sure.
