TPA3110D2: Output clipped at 2V

I am using TPA3110D as Class D amplifier to amplify a 90 Hz sine tone. But my output is getting clipped at 2V. I checked PLIMIT pin, the voltage there is at 6.5V, GVDD pin is at 6.9V. First I used power supply of 12V, then 16V but still output remains clipped at 2V.

Kindly guide me in rectifying my mistake. I want 15W at the output (speaker of 4 ohm). Shouldn't I expect 7.74V peak at the speaker? What should I do to get 7.74V(15W) at the speaker?
