TPA2012D2: TPA

Hello, we use TPA2012 to drive 1W speakers. I am attaching the circuit diagram below. IC is 5vdc supplied.  It seems working but sometimes it gives continuous 5VDC to the speakers for no reason and speakers are heating so much so they melt the plastic case that  we fit them in. Also IC breaks down sometimes because of the current which the IC tries to provide for speaker while output continious 5VDC. Our products come back for this reason. How can it be? Any suggestions? Thanks.

    Welcome to E2E, thanks for your interest in our products.
    The schematic looks well, but the outputs just have connected inductors, so you have to add output capacitors if you are going to use output filters.
    I need more details about your application in order to help you. Besides, could you provide me your PCB layout to review it?
