We have been contracted by Kitron EMS to collect material declarations on Inventory of Hazardous Materials (IHM) Declaration Excel form, which needs to be filled out by its suppliers to demonstrate compliance to Environmental Compliance Regulations.

If the material declaration in the IHM document is same for all the parts covered (for the attached part list), please fill out one IHM declaration form.  If there is difference in the material declaration content, use one IHM form for each part.

Please submit completed material declaration ASAP.

Below files are attached to this email:

·         A blank IHM Declaration Template Excel Form.
·         A Parts List identifying which Parts require a IHM Declaration to be provided.
·         A Letter of Authorization from the client
