DAC101C081: DAC101C081 burn down

Our solution is to drive four groups of loads with DAC + op amp, and the load is led driver + LED. As shown in the figure, a batch of such PCBs have been made, but most of them are burnt out within two months（DAC101C081  burn down）. Please help to analyze the causes?

    Hi Zhang,

    I do not see any obvious issues with the schematic or the layout that could cause this issue.  How are you certain that the DAC is not functioning? If you remove U12, does the output still not work? Are you certain that the supply is 3.3V at all times? Are there power up or power down transients that could be exceeding the abs. max table?
