DLPA200: Tantalum Capacitor Failures with DLPA200

We have had a recent rash of DAD2000 Voltage Boost Converter Circuit Capacitor failures, specifically the VBIAS and VRESET filter capacitor, specified to be 1 to 10uF, 35V, 1 Ohm ESR. We are using Vishay TR3C335K050C1500 – 3.3uF 50V Tantalum on several designs were the DAD2000 is used. We had a FMA for the Visahy cap performed with results from Vishay stating -dielectric breakdown due to "Excessive forward voltage". However we are not seeing any excessive voltage during normal operation or power up and power down.

My question, has TI had any reports of this issue with Vishay Tantalum capacitors? Any insights to our issue.
