Part Number: DLP4500
Does TI supply an Evaluation kit with an FQD type device rather than the FQE(which is what seems to be on the available EVM)? Additionally, if custom optics are required, is it possible to unmount the existing projection optics from the evaluation module, or should I be purchasing a stand-alone controller board and DMD.

TI does sells a complete EVM with optical engine. TI dos not sell stand alone controller board. Changing form one package of DMD to another will require a different flex cable to connect the board.

Thanks for the follow-up. As I mentioned, I wanted a complete EVM that uses the FQD type package rather than the FQE supplied in TI's standard evaluation model. Additionally, I wanted to understand how easy/hard it was to modify the full EVM sold by TI with custom optics.

TI does not have DLP4500 EVM with FQD package.
Replacing complete optical engine with custom optics will be easy. Make sure that you are using DMD with package other wise mounting hardware and flex cable will not connect.
Just replacing an optical component in side engine will be challenging and you will require expertise in optics.
