XIO1100 No Signs of Life

For the XIO1100 chip, we have a non-working prototype PCB. The XIO1100 has no RX_CLK output signal, even though it has 3.3V and 1.8V and 1.5V power, and the two REFCLK input signals are good (100MHz from PCI Express connector). Balls L12, L13, and H11 (various "COM" signals with external bypass capacitors) are all floating (not receiving any power). The seven control input signals are all valid per your datasheet. What could prevent RX_CLK from being active?? What could prevent the three "COM" signals from having voltage present (3.3V or 1.5V). Thanks for your help...
