Evaluation board for TUSB8041: Ports 1&2 failed for "device descriptor request failed" and Ports 3&4 functional. Note that SW1 and SW2 all are set to default (as shipped) which is OFF positions. Same USB sticks functions at ports 3 &4 and multiple computers w/o the TUS8041EVM boards.

Have gone thru the steps of unistalling devices 1&2, checking for new drivers, chkdsk/reboot/cclean etc/, turn on fast startup, changing USB selective suspend setting, Will all these steps , problem still persists for downstream ports 1 &2.

Can you download the usb device tree viewer and send a screen shot of the USB tree when the devices fail to connect? All port should behave identically. Is this a new EVM?

This is a new board we purchased it in early october. It failed almost OOB. See attached doc for all ports

Very strange. All ports should be tested before shipment. Can you send a photo of the EVM? Where was it purchased?

Can you check if any speed devices works on the port ( mouse / USB 2.0 flash driver / USB 3.0 flash drive) if possible? Do the LEDs for power turn on for that port?

See attached photos. I tried few mouses/different USB 2.0. I dont have USB3.0 at this moment. All ports LEDs for power are on. See attached photos. Purchase was from Digi-Key
