TPD4E02B04: Peak pulse power and current(tp=8/20us)

Part Number: TPD4E002
Hi team,
My customer used TPD4E002 on their 485 interface for ESD protection. The EE is motor driver Encoder.
They found 10 pcs TPD4E002 broken after product 1000 boards and working. TPD4E002 broken is meaning that the IO pin close short to GND.
I guessed there are two reasons causing this problem.
1. TPD4E002 Peak power P(Surge) is no enough.
2. When Encoder working, it close to motor running and temp is hot. Too hot to let TPD4E002 broken.
DO you think two reason which one more make sense?
And do you have other suggestions?
Thanks!

You said this device was on an RS485 signal? How high was the signal voltage?
