Whether the 3-pin level is high or low, the 4-pin output is always low.

Could you please help confirm what caused it

    Hi Emily,

    Your first two pictures didn't render properly. Could you please post them again?

    Also just to clarify you are giving 5V to the power supply? What voltage levels are you applying to the inputs? Where does the output signal (A_OFF_ in the circuit diagram in your document) route to?
