Problem with High Voltage Digital Motor Control Kit

I have just started to work with this HV developer's kit: part no. TMDSHVMTRPFCKIT with "HVBLDCMTR" motor.  After following the Quick Start Guide to configure the board for AC power input, I ran the GUI program "HVMTRPFCKIT-GUIv1.exe" that came with it for a quick out of box demonstration.  But everytime I click on the 'Start Motor', the motor locks up or when it tries to rotate, it is extremely rough and jerky. This condition continues for a couple of seconds until it blows the fuse on the board.  Has anyone seen this problem before? I have also ran the HVBLDC_Sensorless source code in debug/ram mde and looked at the pwm signals.  All 6 outputs are perfect square waves, but once I applied the AC power, the signals become extremly distorted and lose its shape. Does anyone know where the problem might be?
