DRV8876: Simulation of Independent Half-Bridge Mode

I am using the provided SPICE model for the DRV8876 in independent half-bridge mode; PMODE is floating.  A pulsed voltage from 0 to 3.3V is applied to the IN1 and IN2 pins to drive resistive loads connected from OUT1 and OUT2 to ground.

The resulting load voltage is steadily just below the voltage supplied to VM (~14 V) instead of being a pulse as expected. Placing the INx pins at ground yields the same output, also. 

I suspect I may not be using the model correctly, any suggestions?
