The leakage current of DVDD while device is in sleep mode and PVDD=13.5V is 2uA typical. So no switch is needed.

The IC does not have an internal DVDD switch. So if you want to disconnect DVDD from the driver, you can use a switch to reduce the leakage current when the device is in sleep mode.

The test conditions id V_PVDD=13.5V and T_J=25C. This specification is only applicable for the current through the DVDD pin. The excess current from the GAIN resistor is not included in this specification.
The pin diagrams are shown in section 7.3.7 of the datasheet.
