BQ24610: BQ24610 high side FET broken

I need more details than it just broke. What is the schematic, what are the operating conditions, etc.?

Attached the schematic for your reference.

At present, during trial production, four boards were found to be broken down and the BQ24610 were damaged. The 30V/15A charging MOS and P-MOS uses 30V/6A.

It was found that four chips were damaged, and the charging MOS tubes of the four boards were all damaged (Q4, Q5)

Can you please send a more clear schematic? it is unclear which nodes are connected or just overlapping. Is it possible to also have a schematic with different contrast?
