TPS63802: Becomes hot when enabled and stops working.

We have a TPS63802 based 3.8v power rail design for powering Ublox SARA R4 GSM module.  It works well and gives 3.8v output.  The input is a LiPo battery 3.7v. However we observe that the TPS63802 goes bad suddenly and gets very hot when enabled.  When it gets hot it will never work. We do not know exactly when this happens but this has happened on multiple boards.

Below is the schematic of the 3.8v rail in our design. Is it possible that the TPS63802 blows up if the load current exceeds 2A?
