TPS56339: IC damaged at SW pin

Hi Team,

TPS56339 IC got damaged at end customer side. Below is the SCH and customer verified there is no spike(positive or negative) happened on SW pin. Customer would like to go FA process and want to know what experiments he can do to find out the root cause?
