LM5064EVK: Hot swap controller

Hi, I am designing hot swap controller  to connect to the back panel of our system.

My requirements are Vmin=-15.075v, Vmax=-14.925v, Vtyp=-15v at 150ma current limit. which eval board or IC should i use for this.

I tried looking at LM5064 eval board but it doesn't support -15V it's out of range but the IC data sheet from -10V to -40V

Is there a single Eval board or IC that can handle both +ve and -ve voltages. My requirements for the system is

1. Vtyp= 5.0v ; Vmin = 4.975v; Vmax= 5.025v at 80ma current limit

2.  Vtyp= +15v ; Vmin = 14.925v; Vmax= 15.075v at 150ma current limit

3.  Vtyp= -15v ; Vmin = -15.075v; Vmax= -14.925v at 150ma current limit

I need under voltage, over voltage and current limit protection for the device.

Please can you suggest and eval board or IC that can be used for the above spec.
