Hi,

Where did you get your schematic from? You can use Webench to get a good schematic. You are missing an input cap, for example. This is required.

But again, I recommend using newer, smaller, cheaper devices instead. It looks like this is a battery powered application, which needs just 750 mA? This IC is not a great choice for this due to its large current, large size, and high Iq. The TPS62824 would be a much better choice.
