Note these materials cover two ports and an adapter, but you can implement the materials with a single port.
Additionally, the challenge will be the 30V adapter. You will need a wide-Vin design since most DCDC transformers are only made for standard PoE range 37V-57V. The transformer will be bigger and have a lower inductance.
We have some wide Vin designs I can share with you in private messenger if you can accept my connect request and tell me you output voltage/current requirements.

1. I did take look title "Delivering a smooth transition from AC to POE power which is good for us ,but I could not find the reference design as that section,Would U please provide it?
2.About DCDC transformers that didn't problems for us,The transformers input range 27V~58V as we design.
3.Our output voltage is 12V@6A

2. A transformer that supports 72W output and a 27V-58V input is going to be very large and probably have thermal issues.
I would suggest using a buck-boost converter to boost your adapter voltage to 48V or 55V and then use it with the design I will send you in private messenger (please accept my connect request). Otherwise you will need to make a custom transformer, and I foresee it having the problems stated above.
